If they are still the same as my one for a "plain" radio OEM on a 1999 Passion then it will be a piece of card about credit card size. One side will be Black with SMART written on it. The other side is silver with SMART written on it and the following:- The Model type = Radio (which ever one it is), Serial Number of the unit and the code.

Sorry no access to a digital camera at present so can't upload a picture.
